The HUBCHART app is a free, HIPAA-compliant, and patient-centered collaborative care app. It serves as a highly effective tool for managing large-scale patient care groups. With the app, care teams can securely share vital information with one another at any time, from anywhere. This team-based approach to care delivery helps streamline and expedite the process.

The app offers "patient-named" collaborative chat groups that function as ad-hoc health records. Within these secure groups, care team members can share chats, files, images, videos, and links, all of which are archived for future reference. The management of these private group chats is handled by a group owner and administrators, as well as other approved participants within the care team. The care team can consist of doctors, doctor's staff, patients or family members, pharmacists, and more.

HUBCHART's patient-centric groups are particularly valuable for complex medical care, case management, and concierge medicine. They enable a multidisciplinary team to continuously coordinate and provide personalized care in an optimized manner.

In addition to improving healthcare outcomes, the HUBCHART app allows providers to bill for care coordination using chronic care management billing codes and other non-face-to-face billing codes.
